  - (Defined As: who received two doses of either the Moderna or Pfizer COVID-19 vaccines
  - OR who have received a single dose of the Johnson & Johnson vaccine, 14 days or more ago)
  - and who do not have symptoms.

  - ... All travelers must complete the Traveler Health Form unless the traveler had left New York for less than 24 hours or is coming to New York from a contiguous state.
  - COVID-19 Travel Advisory: New York State
    - All travelers must complete the NYS Travel Form unless the traveler had left New York for less than 24 hours or is coming to New York from a contiguous state (i.e., Pennsylvania, New Jersey, Connecticut, Massachusetts, and Vermont).
    - It looks like the form must be filled out online, on 8 or nine different pages or screens.
    - QUESTION: When should this form be filled out ?
    - ANSWER: The day before arrival.
  - QUESTION: Do I need to do anything before I arrive in Florida?
